Transfer & Re-Entry Applicants

Each year we accept applications from transfer and re-entry candidates.

Thank you for your interest in the Postgraduate Medical Education program in Public Health and Preventive Medicine (PHPM) at the Dalla Lana School of Public Health, at the University of Toronto. We are the largest PHPM program in Canada and are accredited by the Royal College of Physicians and Surgeons of Canada.

Transfer and Re-entry selection occurs between February and March of each year.

Please note that PGME makes the final decision as to whether a residency program will be granted transfer/re-entry residency positions.

 

University of Toronto PHPM Residency Program Goals:

Our program graduates Public Health and Preventive Medicine specialists with the knowledge and skills needed to assess and manage health issues faced by communities and populations. The program will ensure that all graduates can make independent, evidence informed, community responsive, accountable and ethical decisions that maintain and improve health overall and reduce health inequities.

The program will prepare all trainees to be able to serve as local medical officers of health. However, recognizing the need for Public Health and Preventive Medicine specialists in many roles and the variety of career paths graduates may choose, the program will support trainees to gain clinical certification in Family Medicine and to have opportunities to enhance knowledge and skills in focus areas such as research, education, environmental health, public health administration and global health.

The program will provided residents with learning and assessment opportunities that meet Royal College, program and personal objectives. We support residents' self-direction and self-assessment as well as provide some flexibility in the demonstration of achievement of competency.

Please note that the process described below is for University of Toronto Residents applying through the transfer process. The University has a set policy regarding transfer candidates. The University’s policy for transfers can be found here. Residents from outside of the University of Toronto should contact their postgraduate office. Provincial policies can be found here.

Required Documents

PART 1 *Please follow the same guidelines for the personal letter as described on our CaRMS page, but please state your reasons for wanting to change residency programs.

PART 2:

Once we receive your "part 1" documents, we will inform you of the next steps which may involve requesting the documents below. Please review our CaRMS page, as the process is very similar. Should you be invited for an interview, it will be held in the last week of February.
  • 3 Letters of Reference: At least one from a PHPM specialist
  • Unofficial Post-secondary transcripts
  • Official medical school transcript
  • Dean’s Letter
  • All ITERs to date, in pdf format. Please combine into one pdf file if possible.

  • A copy of your CCFP certificate
  • A certificate of professional conduct from the College of Physician and Surgeons in your province/territory of work
  • All ITERs, evaluations and assessments from all post-graduate medical education, in pdf format. Please combine into one pdf file if possible.
