Transfer & Re-Entry Applicants

Each year we accept applications from transfer and re-entry candidates.

Thank you for your interest in the Postgraduate Medical Education program in Public Health and Preventive Medicine (PHPM) at the Dalla Lana School of Public Health, at the University of Toronto. We are the largest PHPM program in Canada and are accredited by the Royal College of Physicians and Surgeons of Canada.

Transfer and Re-entry selection through PGME occurs between February and March of each year. However, if you are interested in pursuing transfer or re-entry into the PHPM residency program, please contact the program coordinator and program director as soon as possible.

Please note that PGME makes the final decision as to whether a residency program will be granted transfer/re-entry residency positions.

Please note that the process described below is for University of Toronto Residents applying through the transfer process. The University has a set policy regarding transfer candidates. The University’s policy for transfers can be found here.

Residents from outside of the University of Toronto should contact their postgraduate office. Provincial policies can be found here.

If you are interested in pursuing a transfer into the PHPM residency program, please contact the program coordinator and program director as soon as possible. You may be required to provide the following documentation:

PART 1

  • Transfer Application Form
  • Personal Letter (Please follow the same guidelines for the personal letter as described on our CaRMS page, but please state your reasons for wanting to change residency programs.)
  • CV
PART 2: Once we receive your "part 1" documents, we will inform you of the next steps which may involve requesting the documents below. Please review our CaRMS page, as the process is very similar.
  • 3 Letters of Reference: At least one from a PHPM specialist
  • Unofficial Post-secondary transcripts
  • Official medical school transcript
  • Dean’s Letter
  • All ITERs to date, in pdf format. Please combine into one pdf file if possible.

If you are interested in applying to the PHPM residency program through re-entry, please contact the program coordinator (phpm.progasst@utoronto.ca) and program director. You may be required to provide the following documentation:
  • Personal Letter
    • Please follow the same guidelines for the personal letter and reference letters as described on our CaRMS page. However, for the personal letter, please state your reasons for wanting to change careers.
  • CV
  • 2 Letters of Reference
  • Unofficial Post-secondary transcripts
  • Official medical school transcript
  • A copy of your CCFP certificate
  • A certificate of professional conduct from the College of Physician and Surgeons in your province/territory of work
  • All ITERs, evaluations and assessments from all post-graduate medical education, in PDF format. Please combine into one PDF file if possible.
